New Incident Report Form for Student Use
The Office of Student and Alumni Services (OSAS) is pleased to release a new resource for MGH Institute students, an online Incident Report form. Based on students’ requests for a streamlined reporting mechanism, this form has been carefully designed to enable students to describe a concerning experience or an apparent violation of MGH Institute policy in their own words, and submit relevant details easily. It is accessible by phone, device, or computer. Students can also submit reports in person by visiting OSAS in Building 39.
All reports will be treated with utmost confidentiality. OSAS staff will follow up on all reports received. I appreciate the input that students, faculty, and staff offered as we drafted this form.
